Making an Appointment at one of our Offices In order to better serve our clients, NMCRS Hawaii operates on an appointment basis. Although emergency walk-ins are seen without appointments, we ask that you call and make an appointment whenever possible. Appointments assure that you will be seen in a timely manner and reduce your time away from work. |

What do I need to bring to my appointment? In order to make your appointment more productive, you should bring your LES, a list of your bills and expenses, and whichever of the following is applicable: · Basic living expenses such as food, rent, utilities. Bring a copy of your lease, your current utility bill, and any other pertinent paperwork. · Transportation, emergency leave, and other types of leave. Bring a copy of your leave papers, the American Red Cross message verifying the emergency, a doctor's recommendation if appropriate, airline travel itinerary and cost, and any other pertinent paperwork. · Vehicle repairs. Bring a written estimate, your driver's license, registration papers, proof of Hawaii no-fault insurance, and any other pertinent paperwork. · Vehicle insurance. Bring quotes from your insurance company, your driver's license, registration papers, and any other pertinent paperwork. · Medical bills. Bring bills from your doctor describing services performed, insurance reimbursements both received and expected, and any other pertinent paperwork. · Dental bills. Pick up our dental treatment forms before going to your dentist. Bring the completed form to your appointment. You may be asked to have the treatment plan reviewed by the Navy Dental Clinic. Bring any additional pertinent paperwork. · Lost or stolen funds. Bring a police or base security incident report as well as any other pertinent paperwork. · Baby Budget Class. Husbands and wives are encouraged to attend this valuable class together. Bring your LES, a list of your monthly bills and expenses, and all of your questions. · Other. Bring all forms and documentation which support your request for assistance.
NMCRS Assistance is provided on behalf of the active-duty sponsor. Whenever possible, the sponsor must be present for appointments. Servicemembers should complete and sign an NMCRS Authorization to Assist Form and/or Power of Attorney to Facilitate assistance to family members in their absence. |
